Betta Healthcare staff will meet and greet the service user well before discharge, thereby familiarising themselves with the hospital’s specific care needs and if necessary get service user-specific training. The process also provides the care team with an opportunity to learn about the client as an individual, including the client’s likes, dislikes, preferences, and wishes about their life once they are back at home. 

Our hospital discharge facilitators work closely with service users and their loved ones to minimise unnecessary hospitalisation or delayed discharges.
